On the Cover: Images of the right shoulder of a 64-year-old man with large rotator cuff tear showing hidden recess under the subcoracoid space with loose body. When we perform routine examination in the subacromial space, we sometimes do not go further to the space under the coracoid (C) and anterior surface of muscular portion of subscapularis (SBm). However, frequently there are adhesions or bursa hypertrophy between the coracoid undersurface and the subscapularis, and a large amount of bursal or joint fluid can be captured in this area. If we just repair the supraspinatus-infraspinatus and even the subscapularis without releasing the adhesion between coracoid and the anterior surface of the subscapularis muscle, we may be just leaving this fluid and any loose body inside.
